You have a big mess on your hands. Most of these codes indicate a poor air/fuel mix. Buy a can of Amzoil Foaming Cleaner. Use it to clean the throttle body and plate and IDLE AIR CONTROL VALVE. It may take two people to do this. One under the hood, and the other in the car. Remove the cover to the throttle body, and have the friend push the gas pedal to the floor. Squirt the Amzoil through every passageway you can find. Follow up with a Gum-out chaser. Do this first with engine off. Use these cleaners to clean the PCV valve and hose. Now, start the van and alternate the cleaners through any vacuum ports you can find, including the PCV valve. Eventually, these cleaners will remove the muck inside the intake manifold and blow the debris out the tailpipe. The O2 sensor(s) will get cleaned and return to normal functioning. For best results, remove the factory junk plugs and install a set of Bosch Platinum or Fusion plugs. Even better, Nippondenso Iridium plugs. You will be amazed at the savings in fuel, and the increase in power!!!! It is dramatic. Expect 15-20% improvement. You may have to have the check engine system reset. Auto Zone does it for FREE! Worst case scenario, go to dealership for a GM reset. They'll charge you, though.
